Unpaired Z-wave dimmer, now can't pair it back

I have several Z-wave dimmer switches paired to my Vera.  I am using a Logitech Harmony Hub with a hub extender to control the dimmers.  I added a new GE Z-wave dimmer to my room, so I paired it with the Vera, expecting the dimmer to show up in my list of devices on the Harmony.  I was not able to get the new dimmer switch to show up on the Harmony, so I wanted to try unpairing and then re-pairing the new dimmer with the Vera.  I successfully unpaired it, but then when I went to pair it back, the Vera never sees the dimmer.  I followed the same procedure that I used when pairing the first time (pressed the off button on the dimmer switch once).  I tried updating the Vera to the latest firmware, but that had no effect.  I also tried resetting the new Z-wave dimmer, but that didn't help either. 

Any ideas on what I might need to do to add this Z-wave dimmer back to my Vera?  Thanks.

Two thoughts:

1) I would try a full reboot.  From settings / net & wifi / Reboot

2) perhaps the device was not fully removed from Vera.   Try going to the device log, located at:

http://192.xxx.xxx.xxx:3480/data_request?id=user_data&output_format=xml

If you find the Device ID of the "deleted" device you can Remove with this:

http://192.xxx.xxx.xxx/port_3480/data_request?id=device&action=delete&device=zzz   where zzz is the device to be deleted.
